The Sky Jewel is one of three rare gemstones needed to craft the Travel Stone. The item has no other uses or purposes beyond crafting.
You can only get the Sky Jewel by winning the Grand Prize at Prize Palooza, a lottery stall that becomes available at Bazaar Rank 5 after you complete the request “A Prize Stall at the Bazaar“.
Each drawing costs a set amount of Medals:

The Grand Prize can only be won once per save file, but there is a built-in pity system: if you draw 50 times without winning the Sky Jewel, the next drawing you do is guaranteed to be the Grand Prize.
The Travel Stone is crafted at the Yellow Windmill with the Purple Wonderstone active, and with all three gemstones:
